That rule has always been contradictory with the rest of the rulebook.  For example:

5.3. Any target engaged with the use of an active laser or weapon-mounted light will incur one (1) PE for each target using an active laser or weapon-mounted light


The intent of the rule (not piling on for single mistakes) is noble, but IMHO it's the most awkward part in both versions of the rulebook.  It requires PEs to have meaningful types, but there's no clear enumeration of types.  It means there is no clear answer for how many PEs have been earned in the following scenario:

Stage Procedure: Start at P1. Advance to P2. Engage T1 & T2 from P2.  Proceed to P3. Engage T3 & T4 from P3.  All targets 2 rounds each for 8 total shots.
Shooter goes from P1 to P3, Engages T3 & T2, then moves to P2 and engages T1.  2 on each target for 6 total shots.
How many PEs did the shooter earn?

I want to say 4:
- Sequence error slicing the pie at P3 and exposing himself to T4
- Visited the positions out of order
- Engaged T2 from the wrong position
- Failed to engage T4

An argument can be made for 5:
- Sequence error slicing the pie at P3 and exposing himself to T4
- Visited the positions out of order
- Engaged T2 from the wrong position
- Failed to engage T4
- Failure to fire the required number of rounds according to the stage procedure.

A very good argument can be made for 2:
- Sequence error slicing the pie at P3 and exposing himself to T4
- 3 (or 4) instances of failing to follow the stage procedure, reduced to 1 PE by 5.1.2.

There is nowhere in the rulebook that definitively resolves this.
